MASTER ACCOUNT AGREEMENT TERMS AND CONDITIONS

This is a credit application and agreement with Valiant Home and Energy of 165 Railroad Hill Street, Waterbury, Connecticut. By checking below you agree to allow Valiant Home and Energy to perform a credit check. Valiant Home and Energy may cancel your account if any information provided herein is materially false or if you default on this account.

By checking below, you are agreeing to the terms below:

• Valiant Home and Energy maintains one master account for each customer. To participate in any of Valiant Home and Energy's special programs your master account must be in good standing.
• All account balances are due and payable within fifteen (15) days from the date of billing (exclusive of budget accounts not in arrears). In the event any extended payment plan is agreed to (for equipment installation, service, tank removal, etc.), you will be required to make agreed upon reasonable periodic payments at least once every calendar month. Past due balances of thirty days or more will incur a finance charge of 1 1/2% per month. Additional fees may apply.
• All other programs offered by Valiant Home and Energy for your participation (i.e. budget, service, etc.) are subject to the terms and conditions of that program and will become part of your master agreement.
• All Services and Service Plans are billed in a separate billing system from your deliveries. You acknowledge that invoices for Service and Service Plans are emailed from Valiant Home and Energy and payments are to be made from these emails.
• In the event that any checks are returned for any reason including and not limited to insufficient funds, closed checking account, or any other reason, your Valiant Home and Energy account will be charged a returned check fee not to exceed $50.00. In the event a customer defaults on this account agreement Valiant Home and Energy may resort to legal collection procedures. You agree to be liable for all reasonable costs of collection and attorney's fees as limited by law and the court.

I accept the terms and conditions in this document and understand that Valiant Home and Energy will establish one (1) master account in my name subject to the terms and conditions included herein.
